Is there anyone who is billing the EM Level's New/Established 99201 - 99215 on the UB form which would be willing to share their policy on determining New/Established and Level 1 - 5 assignment?

I have never heard of billing E/M visits on a UB form. Are you sure it isn't CMS 1500?
