Procedure Performed:

Utilizing sterile technique and ultrasound guidance, the left posterior tibial artery was accessed retrograde with
a micropuncture needle following approximately 5 cc of 2% lidocaine. Ultrasound is initially used to confirm the patency of the posterior tibial artery. Utilizing microwire exchange, 5 French micropuncture dilator was inserted. Next, utilizing Jwire exchange and serial dilatation, a 5 French slender sheath was advanced. Next, a 4 French rim catheter is advanced where angiography of the SFA, popliteal artery and tibial trifurcation was obtained.
Findings: Hyperemia to the medial aspect of the left knee joint was noted along the pathway of the descending medial genicular artery. The superior medial genicular artery was diminutive in caliber. Slight prominence of hyperemia was seen along the lateral knee joint via the superior lateral genicular artery.
These were carefully subselected with a 2.1 microcatheter as well as a true form microwire. Each of these vessels are subsequently embolized utilizing standard technique.
Embolization: Ice is applied to the skin. Embolization is performed with Primaxin utilizing the standard admixture. Careful attention is paid not to obtain nontarget embolization.

I agree with 37242 and 76937. Since he started in the posterior tibial artery, I would code 36245 x 2 for the genicular arteries, no 75710 as this is not a diagnostic exam.
