In a provider based hospital setting, if an EMT places an arterial cannula under the direct supervision of the MD, can the physician bill for the supervision?
 
No. Incident to guidelines do not apply to EMTs. Additionally, incident to may not apply in the hospital environment--depending on some specifics. What you are describing does not sound like it would fall into the category of incident to allowance.

The EMT performs the service without any professional cost to the physician; too, the EMT is not a provider who generates professional services. Therefore, you may not charge for this service.
