I had a patient's wife come into the office to discuss with the doctor end of life care without the patient in the office. I originally billed medicare with the code 99214 and the initially paid for it. But now I got an error from Medicare stating "the procedure code/type of bill is inconsistent with the place of service. Missing/incomplete/invalid/inappropriate place of service." At the time the wife came to our office the patient was admitted to the hospital. Speaking with my office manager we are thinking that we need to bill 99497. Is that correct? Would I need any modifiers?

We generally will bill this type of service under 99497/99498 for Advance care planning. This code can be used for face-to-face with either the patient or family members or surrogate.
