When 2 bx's are done, snaring colon polyp - adenomatous and cold bx of colon mass - malignant. (the pt is with anastomosis and hx of colon ca back in 1998) Is it right to charge for both or just for the "significant findings"?

It is really hard to give good coding advice without the op report you are referencing, but, based on what you stated - you can report separate methods if they are performed on separate areas or lesions. If they are reported on the same lesion you can only report one of them. So - if the polyp was removed by snare in the splenic flexure and the malignant mass from the rectum was biopsied by cold forceps then yes - you can report both.
