the provider performed endoscopic anterior ethmoidectomy and excision of left concha bullosa. can i use cpt codes 31240 and 31254

he microdebrider was then used to take down the bulk of the visible polyp. It was then seen that this was coming from the anterior and lateral aspect of the middle turbinate. CT imaging had also demonstrated an osteoma from within the middle turbinate or ethmoid area. A hole was created in the anterior hollow tip of the left middle turbinate. Jansen–Middleton scissors were then used to make a cut on the superior and inferior aspects of the turbinate. Once this was done, Metzenbaum scissors were used to make posterior cuts. This then allowed for removal of the lateral half of the turbinate along with polyp. As this was done, the mucosa so that was redundant within the concha was removed. Further dissection exposing part of the anterior ethmoid cells showed a bony mass. This was carefully dissected free of the mucosal attachments. This was then sent separately for pathologic examination. The microdebrider was then used to remove redundant areas of mucosa and bone on the cephalad and inferior parts of the middle turbinate.
 
we have been billing 31240 with 59 and lateral mod, some get pd and some don't, don't forget that in past years no middle turb work was allowed as it was considered access. I think more and more insurance companies will be denying that code as included once again and it has erupted over the last few years and more than likely over used. Your case sounds like an appeal due to the osteoma
