Wiki ENT coding question

We billed 14061,14041, along with 15004 X2, medicare paid for the 14061 and 14041 but denied the 15004X2 as the number of units exceeds our acceptable maximum. does anyone have any advice on getting paid for the 15004.

Are you prepping two different sites or just one? It it's one then use the add on code. If its more than one site, use -59?
