Procedures performed were 31296, 31295, 31297, 30140 & 30930. What justifies an office visit with a 24, 25 modifier with a nasal endoscopy with a 79 modifier. Same DX's as the nasal surgery. Also seen office visit (25) and an endoscopy. My thoughts are postop visit with endoscopy, 58 modifier. What are the guidelines? Any suggestions? Thanks
