Hello,

I code for EP Ablation. I am desperate if anyone could please send me a list of insurances that require precerts or referrals for these procedures.

I would greatly appreciate it!!!

Referrals or authorizations is based on the patient's plan, PPO, EPO or HMO. Since coverage varies from state-to-state, this info can be found on the insurance payer websites, usually under their benefits section. I know its time consuming to research but this is the best way to verify what plans need referrals/authorizations. Medicare does not require authorizations.

Being in claim department, I can advise you to contact provider service line they have the list of codes that require precerts or not.
