So can I only bill: 93618-26,93600-26,93603-26/59,93612-26/59?

93609 can't be billed as it is an add on the ablation code or 93620
93621 can only be billed with 93620.
 
Is there any way you could post the OP report? I need to see a little more of what the doc did.
