Doc states he performed an "epidural steroid facet injection on L4-L5." He also uses fluoroscopic guidance. I'm new to this and confused as to code which of the following: 64483, 64493, 62311
 
:confused: -TC modifier is for the facility. -26 is professional services. Does the provider own the equipment? That will make a difference on which modifier to use.
 
We use 64493 for the "facet" injections which includes the fluoroscopy for single level lumbar. If done bilaterally, modifier 50 is attached as well.
