Hello everyone,
need help billing for a medicare patient with anemia due CKD stage III, is hcpcs Q4055 and 96372 the only ones i need? :confused:

not sure which part of the country you are covering, but my MAC has a specific LCD for this -- L33617: Erythropoiesis Stimulating Agents (ESA). Best to check with your local carrier to insure correct submission.
