I have a question concerning the screening questionnaires associated with CPT codes 96160-96161. I am being told that as long as the results of the screenings are documented in the encounter, then the forms themselves do not have to be uploaded/scanned into the patient's chart. I have searched and not found any source of truth mentioning the form in any way, but would like to verify before going back to this provider and telling them that they are okay.
