It is impossible to tell you what to code the second visit without reading the documentation. cc, hpi, exam, mdm. why did this patient come back a second time?

it depends on whether you code for the physician or the facility. If you code for the physician then you cancombine both encounters together and bill one level. If you bill for the facility you bill another visit level for the second encounter and append the 27 modifier and use a G0 condition code on the claim.
