I have a question regarding coding ER injection charges for a patient who ends up being admitted (inpatient) and has surgery.

For example, a patient comes to the ER with severe abdominal pain. He is given IV meds for pain and sent to x-ray. After radiology results are returned and patient has been examined, it is determined that the patient has appendicitis should be admitted (inpatient) for surgery. Do we charge for the administration of IV pain meds that were given in the ER or is that part of the surgical package?

What if the above ER patient were sent to surgery as an outpatient (same day surgery or observation status)?

Any insight would be appreciated.

In lieu of a specific facility guideline, we end all ED facility services when the provider writes the order for inpatient or observation services. Most of my recent facilities want all injections and infusions billed until the patient leaves the ED.

Your facility should have a policy regarding when to end ED facility services.

Typically if it is the ER physician ordering and the patient is under his direct care, Serivces would be charged until the time the patient leaves the ER. If the surgeon would come to the ER to evualate the patient and order an antibiotic presurgery and this is a typical part of the surgical procedure then it probably would be included in the surgical code........
