would anyone have any suggestions for what procedure code please?
ASSESSMENT:
Left long finger distal phalangeal crush injury, comminuted fracture with nail avulsion.
PLAN:
Patient was given Toradol 60 mg IM, Norco 7.5/325 mg p.o. Wound was soaked in Hibiclens solution. Adacel was updated IM right deltoid. After soaking, the wound was blocked at the proximal phalanx with 5 mg of 2% lidocaine. Block was successful. Tourniquet was applied during closure procedure by the use of a rubber band. The tourniquet went on 1335 and was
removed by myself at 1338. Wound was cleansed thoroughly and irrigated thoroughly with sterile normal saline wound wash and closed gently with four 5-0 Prolene simple interrupted sutures. Throughout this time, nail was able to be reapproximated somewhat and closure was completed. Covered with Neosporin and Adaptic, wrapped with Coban. Just previous to wound wrap, I did cauterize the nail to allow the pressure of the hematoma underneath the nail to be relieved.
