If a patient is seen in ER, taken to surgery immediately, and then admitted how would you bill? The patient was never shown as Outpatient. My thought is you bill the E& M code under ER, and the surgery under inpatient, even though not admitted until after surgery. Is this correct or should we bill the surgery as outpatient? Thanks for your help.

Before you can code the E/M visit, you need to know the ultimate status of the patient. Was the patient admitted as an Inpatiient? Outpatient? Obervation? It could be any of those. If the patient was actually I/P status, then you would code the ER visit as I/P visit. Until you know what status the patient was on, you can't code this. I have to code this constantly since my docs are called to the ER frequently. It seems odd at first. Just match the ultimate status of the patient. Many surgeries are done on an outpatient basis.
