Hello, I am working a facility claim denial where they have denied all of the charges for denial codes A1 and N464 incomplete invalid support data, the codes are 99285 and G0463 and a variety of lab codes. I am wondering if the admission type of 2 ( Urgent Care ) in form locator 14 would cause this denial for the ER code? Thank you
