I'm a bit confused which encounter to use for Erbitux.
Do you consider it to be a Chemotherapy (V58.11) or Immunotherapy (V58.12).
It is defined as a "monoclonal antibody" which to me says immunotherapy but it is listed in the HCPCS book under the Chemotherapy section.
