Wiki Ercp 43274 and the use of xs modifier

I have a couple of denial from Medicare. ERCP was done and 43274(stent placed) x 3. I billed 43274, 43274x2units with XS modifier.

The denial is for frequency of 43274. Has the use of XS for ERCP's been changed? Is there is a publication that I am missing?

The code has an MUE of 2 which is why its being denied
