I'd code Chronic Kidney Disease requiring chronic dialysis which happens to be the same code as ESRD - apparently.
using Volume 2, Alphabetic Index - looking up:
Disease
kidney
finding "chronic" 585.9
requiring chronic dialysis 585.6

Also looking up under:
Disease
renal
end-stage 585.6

same code "585.6" now, DOUBLE checking via the Volume 1 TABULAR LIST looking under 585.6 it appears that
End stage renal disease AND Crhonic kidney disease requiring chronic dialysis are the same code - 585.6.

Of course I am assuming that they are receiving chronic dialysis..(on going).
