Is the patient established or new when the provider treats them in a setting other than their own office for the first time and then sees them in their own practice? (The initial encounter would be under a different tax ID, different from the provider's practice or individual/personal one.)
 
Face to face in last 3 years to same provider is always established, even if say one practice is in California and the other is in New York. Group practice (defined by TIN) is only taken into account when its a different doctor of different specialty and subspecialty.
