I work for a billing company, and with one of our newer clients some physicians are transcribing the office note the day prior to the visit and then signing it after the visit. Is this acceptable?

If the provider is just setting up a template in the chart and then adding the appropriate details and make corrections to the template based on the visit, then that could be OK. But if the claim is for services on one date but all of the documentation was completed on an earlier date, then there is really no valid documentation that can support the charges on that claim.

Hi, how would that note be accurate and detailed? Im assuming they can predict the patient's chief complaint, symptoms, physical exam findings :) You could probably find something by researching- Progress note and dictation standards.
