Would someone please help me clarify....is there a published guideline about dictation matching the "description" of procedure given by physician? Example: physician states he did "shock wave lithotripsy" in the description of his procedure. In the body of his op report he states "lithotripsy was performed." Do you give him credit for the ESWL or does he have to specify his approach in the body of his report?
