Provider did an exploratory laparotomy, evacuation of intra-abdominal fluid extending from the hernia sac, with closure of hernia sac , , please help

I'm curious to see the op note because if he closed the hernia then it would be a hernia repair and not an evacuation.

I wouldn't code the evacuation of the fluid if it's a hernia repair, its inclusive.
