As a student learning just learning coding, I am stil trying to figure out how to disect the Diagnosis or the History, Exam, Decision Making parts.
For example.....A patient was given instructions on taking Metamucil. After taking it for a week as instructed, the patient complained of generalized addominal cramps. The patient was instructed to discontinue the Metamucil.
Where is the best place to start with figuring out the History, Exam, Decision Making and the correct code?

What should I be looking for first, second, so on....?
