Wiki EWSL in urinary bladder

I have a provider that performed EWSL on a bladder, and coded 50590, which the payer is denying for needing an anatomical modifier. I am wondering if that is the incorrect CPT code since the bladder doesn't have an anatomical modifier. But I can't find a more appropriate Lithotripsy code? Does anyone have any advice?
 
52317 and 52318 both state "crushing or fragmentation of calculus by any means in bladder and removal of fragments;" so I would choose one of these. Likely 52318 for "complicated" since an ESWL machine was used.
