Wiki Exam done at a later date

Patient was seen for a Bariatric consultation - for some reason the exam could not be completed that day so she was not going to be charged till she came in to complete it. She came in a month later and again the exam was not completed - she was seen for weight loss the second visit. Can the doctor complete the exam for the first visit at her third visit and have it count for the consult at her first visit? Should he date the exam? Feed back would be great! Thanks!:D
