Neurological: He is alert and oriented to person, place, and time.
Steady gait, moves all fours, CNs grossly intact


I count the steady gait and moves all fours as Musculoskelatal and CN's grossly intact as Neoro. Im using the 95 guidelines. Is this right??
 
And what about

And how do you count "he is alert and oriented to person, place and time" (frequently abbreviated as A&Ox3)
