If provider documents "unable to assess" for a bullet point in the exam of an est pt e/m phone visit can this be counted as a point or not?

I am fairly new to coding, but I do not believe unable to access counts unless in the ER and the PT is unconscious. Additionally, I believe to use an actual E/M code, both audio and video must be utilized during the visit. Simple audio is not acceptable but can be billed under other codes.

I believe one of these codes could work for a phone only visit, depending on the insurance:


G2012: brief telephone check-in (established only)
99441: telephone visit 5-10 min (new or established)
99442: telephone visit 11-20 min (new or established)
99443: telephone visit 21+ min (new or established)

G2010: brief asynchronous check-in (established only)
99421: asynchronous e-visit 5-10 min (new or established)
99422: asynchronous e-visit 11-20 min (new or established)
99423: asynchronous e-visit 21+ min (new ore established)

I believe Telehealth coding in emergency situations are 99201-99215 with modifier 95 is like a face to face visit without vital signs. You can find more information in appendix P of CPT book. Except 99211 (can not use it)
