Exam: "no interval change relative to prior visit this month."

I don't know if I can count an exam with this statement ?

Can it be counted if he puts date of service of exam he is referring to..ex.."no changes in exam from prior visit on 10.20.14

no, exam cannot be referenced from a previous visit. Exam is what the physician see/feels/hears at this point in time.
