Hello-

I have a question on what size and CPT code (1164?) you get with the following note. To determine this code it's excision size, which includes size plus margins.

Procedure: Excision
Reason for Excision: excision of a skin cancer
Location: Forehead
Indication: Squamous cell carcinoma
Size of Lesion: 5x6 mm
Size of Excision including margins: 13x14 mm
Size of Closure: 2.4 cm

Any feedback would be greatly appreciated.

Jackie, my answer is 11642 for an excised lesion of 1.4 cm (14 mm)

Remember, excision is based on the largest clinical diameter (6 mm) plus the most narrowest margin (4 mm) required for complete excision.

There are many ways I teach this concept, but if you look in your CPT Professional Edition, there are great examples of how this works.
