Can anyone help me with coding an excision of fibromatosis from a thigh? I'm seeing all sorts of threads regarding plantar fibromatosis, but nothing about the thigh. I coded it as 27327 with dx 728.79. It is now denied for dx/procedure mismatch.
I can't use the benign skin lesion codes since it is involved in the fascia AND the LCD doesn't support this dx for those codes.

Op note states:
A longitudinal incision was made over the palpable thigh nodule and carried down through the skin and subcutaneous tissue. The mass was more fixed in nature and embedded in the fascia over the muscle and was not consistent in character with a lipoma. We therefore sent a specimen for frozen pathology and it came back as benign fibrous tissue. We therefore excised the remainder of the mass with a small amount of adherent fascia and sent it for pathology. We then examined the wound for hemostasis and irrigated it. The wound was closed with interrupted sutures and Dermabond.

Path report states:
Right thigh, excisional biopsy:
- Features consistent with fibromatosis.
- Negative for malignancy.

Any help is appreciated!
