What CPT would I use for the excision of an old fibula fracture fragment?
I'm thinking 27635.

"Then, a linear incision was made from the mid portion of the fibula distally along the lateral aspect of the foot. This was somewhat in between the anterior talofibular ligament and the peroneal tendons. This allowed access to the anterior talofibular ligament, the fibular avulsion fracture fragment, and the peroneal tendons. I first was able to identify the fibular avulsion fracture fragment via intraoperative fluoroscopy. It was not readily identifiable. There was fibrosed scar in between, yet it was somewhat easy to remove via a small rongeur and osteotome. "

Any help is appreciated.

Based on what you supplied, I would not use that code. It states "excision or curretage of bone cyst or benign tumor, tibia or fibula". Neither of these were mentioned in your narrative. You might take a look at the removal codes for loose body or foreign body.
