I have been HCC coding for a few years so I am out of practice with CPT. Provider's note states SCC lesion 1.6 cm including margins, then he has the layered closure dictated as final length 2.4 cm. When I questioned them about the size difference, they stated it's for clear margins. Should clear margins be included in the excision size or can the 2 sizes be different? TYIA
 
so doctor stated the total lesion with the margins is 1.6cm for the excision and the intermediate closure would be the 2.4cm
