I assume that 3 is the largest diameter of the lesion and .3 is the margin ? You take the size of the largest lesion diameter and add the margin.... the margin of .3 would be on EACH SIDE of the lesion diameter so .3 on one side plus the 3 then the .3 on the other side will give you the entire excised diameter.

Its helpful to refer back to the pictures in the CPT book in the lesion removal Guidelines. It will show that you need to double the margin. Think of the margin being part of the radius of the circle. You need to double that to get the diameter. The code measurement is for the complete diameter of the lesion and its margins.
