anyone have a good cpt code to use for a exercise stress echocardiogram for diagnosis codes 272.4 (hyperlipidemia) and 250.03 (diabetes type 1)?
i need to see if this service needs pre certification or not

93351, but the diagnoses are not payable per Medicare LMRP guidelines, which I believe BCBS follows pretty closely
 
Aetna is now requiring precert for them, see if patient has 401.1, think that is payable.
