Wiki Exicision of Benign lesion

Have and excision in which the Dr. states: removed with an ellipse of skin 2.5 cm long. There are not margins to add, are these needed or in anyones opinion has the Dr. added them for the the biller.

Did a little research and am not finding any answers. Am thinking this needs the margins but would like some imput. Thanks in advance.

when a Dr. excises a lesion he has to document the size of the lesion as well as the margins. You then add these all together to get the appropriate CPT. If he is not, you would be stuck billing the lowest code and he'll be losing revenue. Doctors should be forced to take coding courses I tell ya...
