Where does no exophthalmus get marked off in the eyes? Or is it just part of the thyroid exam?
 
According to Wikipedia, Exophthalmos (also called exophthalmia) is a bulging of the eye anteriorly out of the orbit, so I would say you would mark the eyes. I use Wikipedia all the time and have found it to be a very useful resource. It will also give you an ICD-9 code most of the time, if you are having trouble finding the correct one.
