Physician sees pt in ofc for menorrhagia & iron-def anemia. Attempts Hysteroscopy w/ ablation but cannot due to large polyp filling entire cavity. Physician plans hospital procedure and dictates H&P same day of ofc visit.
Four days later, on same patient, physician performs a resection of endometrial lesion, endometrial resection and ablation.
Ins co is UHC
Am I correct in coding as follows:
Ofc visit-
58555
submitting documentation with claim
Hosp procedure -
58563-58
 
I would code the first encounter with 58563-53, then 58563-58 for the second encounter.
I have been OB/Gyn coding for just 6 months, so would like to know more thoughts from a more experienced coder.
