Goodmorning!

I need a little help coding this surgery. Patient returned to OR after exploratory laparoscopy two days earlier.
This is what the surgeon performed.

1.Exploratory laporoscopy with conversion to laporotomy
2.Copious abdominal washout
3.Resection of perforated segments small bowel with side-to-side anastomosis
4.Oversaw serosal tear colon
5.Gastrostomy tube placement.

44120 for the small bowel anastomosis (open)
44602 (small or 44604 for large) for the repair of the tear.. as long as it wasnt caused by the Dr during this session
If they are just oversewing the anastomosis cant add it, I am not sure if you can bill it as i dont have my CCI edits available but if it is a differnt site (like the large intesting you may need modifier 59
the tube could be 43830-43832 depending on the notes

Modifier 78( i am assuming it is a complication of the first surgery) and add the dx v64.41
