What diagnosis code do you use for a 29 year old woman whose mother had a history of recurrent pregnancy loss and believed to have received Diethylstilbestrol (DES)?

Dx code "history of Diethylstilbestrol (DES) exposure"?
 
No I would not since you state it believed to have been DES exposure and it was her mother and not her. Look under family history but you need a little tighter documentation to code even as family history.
