We have a patient who is consistently exposed to silica dust on the job from the grinding of materials, and I'm trying to decide on the best ICD-10 code. I know there is Z57.2, "Occupational exposure to dust," but I'm leaning toward Z57.39, "Occupational exposure to other air contaminants," since it's not just dust, it's a specific (more dangerous) kind. Or should it be a "toxic agent" (Z57.5)? Can I get a second opinion on which of these would be most appropriate? Thank you!

I would go with Z57.2 and not assume anything other than what is stated. Index leads to that -- Exposure to > Occupational > dust. There is nothing more specific after that. I would not use a toxic code unless stated in diagnosis. I don't have your full note, but I would not assume it's at a toxic level (are employer/patient taking precautions & following OSHA guidelines?). Just my thoughts.
