If a physician conducts a level 5 complete/comprehensive History, and a complete/comprehensive Exam for an established patient after our RN has done a Wellness, does this count as a level 4 or 5?
Our MD is doing them as Extended Exams after our RN does Wellness questionnaire and screening discussion.
