Need help guys,

How would you code the following:

1. Bilateral L5 dorsal ramus nerve branch blocks under fluoroscopic guidance with radiologic interpretation.
2. Right L4 medial nerve branch blocks under fluoroscopic guidance with radiologic interpretation.
3. Bilateral L3 medial nerve branch blocks under fluoroscopic guidance with radiologic interpretation.

These are all done on the same patient. I would very much appreciate any input.
 
If done on separate days:

1. Bilateral L5 dorsal ramus nerve branch blocks under fluoroscopic guidance with radiologic interpretation. 64493 - 50
2. Right L4 medial nerve branch blocks under fluoroscopic guidance with radiologic interpretation. - 64493 - RT
3. Bilateral L3 medial nerve branch blocks under fluoroscopic guidance with radiologic interpretation. 64493 - 50

If done same day:


1. Bilateral L5 dorsal ramus nerve branch blocks under fluoroscopic guidance with radiologic interpretation. 64495 - 50
2. Right L4 medial nerve branch blocks under fluoroscopic guidance with radiologic interpretation.64494 - RT
3. Bilateral L3 medial nerve branch blocks under fluoroscopic guidance with radiologic interpretation. 64493 - 50
