Hi

We recently hired a pain mgmt doctor, we are ortho, can anybody give me some guidelines on how many injections, 64493.94 and 95 that Medicare allows per 12 months. Also suggestions on where to find guidelines.

You can go to CMS.gov and look up LCD's for your carrier.
64490-64493 will be under:
Facet Joint Injections, Medial Branch Blocks, and Facet Joint Radiofrequency Neurotomy Policy
Keep in mind that Facet joint injections and Medial branch blocks are coded using 64490-64493.
Many insurance companies have policies on these. You will have to check with them individually.
