Aetna consider facet joint injection (64493-64495) experimental and investigational. I think they only pay 2 sessions during a 3-4 years period. If PT requires injection and wants to pay cash, do we need to have them fill out an ABN?
 
Only Medicare requires ABN. HOWEVER, it is always, always, ALWAYS good practice to have the patient sign a document that contains the items that an ABN does. It will avoid a lot of problems in the future.
