This is a facility bill, mod 21 can not be applied correct?:

Pt who has fluid drained from his/her pleural catheter. After the nurse removes the 400 cc of fluid, they have to observe him/her for 1 hr with frequent vital signs and pain medication. How would we account for that—increase the visit level? Or is this bundled in with the treatment and can not be billed seperately?
