My provider attempted a Novasure ablation, couldn't get a proper seal but was able to attain an endometrial biopsy by endometrial aspirator. Normally an ablation cannot be billed separately with an EMB. Due to the failed attempt, is it ok to bill the ablation with 74 modifier and the EMB separately? I'm not sure what to do with this. Thank you for any help from anyone.
 
In this scenario, I would bill the Novasure with -53. -74 is used for the facility, not the provider. I would not bill the EMB as it is a component of the procedure your provider attempted.
Your claim will likely need an appeal letter and a copy of the op note.
