I was reading the Official CPC Certification Study Guide by AAPC where it instructs to code a failed attempt to insert a stent into a 58-year old female with severe coronary arterial occlusive disease as ICD-9-CM 996.09 (answer to Question 9 of chapter 14). What is the reasoning for this since the stent has not technically been used and therefore could not have failed? Why is there no separate code for failed attempt to insert a device, in the same line as a failed induction of labor?
